Forms for Persons Filing an Appeal (Bill 51)

The following forms are to be used as part of the OMB appeal process and have been developed for common appeals heard by the Board.

The forms may be completed by the appellant or an authorized representative. Please read the instructions carefully and complete the entire form. Some appeals must be filed with the municipality or approval authority and some are required to be filed with the OMB directly. The form will instruct you where to file your appeal.

Please make sure to include any required documents as requested on the appeal form. You must also include your filing fee, paid by certified cheque or money order only. Fees are $125 per person/per appeal. For additional OMB fees please see the OMB Fee Schedule.

Forms must be printed single sided ONLY. At present there is no electronic or e-filing of forms.

Planning Act Appeal Forms

Appellant Form (A1) (PDF) (Word) This form is to be completed by the appellant for appeals relating to minor variance, consent, zoning by-law (passed by the municipality), interim control by-law, official plan or official plan amendment, or subdivision. (To be filed with the municipality or approval authority.)

Zoning By-law Amendment: Holding Symbol Appellant Form (O1) (PDF) (Word) This form is to be completed by the appellant for appeals relating to the removal of a ‘Holding Symbol’ from zoning by-law amendments where the municipality refused the application or failed to make a decision within 120 days. Site Plan Appellant Form (O2) (PDF) (Word) This form is to be completed by the appellant for appeals relating to site plans. (To be filed with the OMB directly.)

Demolition Permit Appellant Form (O3) (PDF) (Word) This form is to be completed by the appellant for appeals relating to demolition permits. (To be filed with the OMB directly.)
