Municipal Submission Forms (Bill 51)

The following forms are for use by municipalities and approval authorities. These forms should be submitted to the OMB along with the appellant forms received and the required attachments.

These forms should not be used by appellants or their representatives.

If you are not with the municipality or approval authority, please refer to OMB Appeal Forms.

Forms must be printed single sided ONLY. At present there is no electronic or e-filing of forms.

Material and information is to be forwarded to the Ontario Municipal Board by approval authority/municipality under the following subsections of the Planning Act, R.S.O. 1990, c. P. 13, as amended. Please choose the applicable form which corresponds to the section under which the appeal(s) have been filed.
