OMB Appeal Forms – (Non-Planning Act Forms)

The following forms are to be used as part of the OMB appeal process and have been developed for common appeals heard by the Board. The forms may be completed by the appellant or an authorized representative. Please read the instructions carefully and complete the entire form.

Please make sure to include any required documents as requested on the appeal form. You must also include your filing fee, paid by certified cheque or money order only. Fees are $125 per person/per appeal. For additional OMB fees please see the OMB Fee Schedule.

Forms must be printed single sided ONLY. At present there is no electronic or e-filing of forms.
